1. One stamp-sized photograph should be affixed to the form.
2. All columns should be filled by the applicant.
3. The filled up form should reach to The Executive Director, TIFF on or before 13th November 2011.
4. Those who apply by post should attach a Demand Draft of Rs. 300 (Rs. 150 for students) payable to TIFF, Thrissur
5. Please attach a self-addressed envelope with 5 rupees stamp to receive your card by post or collect
it from the TIFF office between 15th - 17th November 2011.
